ADP ASSISTIVE DEVICES PROGRAM - BREAST GRANT
ONTARIO MINISTRY OF HEALTH AND LONG TERM CARE (MOHLTC)
Ontario's ADP provides a grant of $195 to Ontario residents every 2 years towards the purchase of a silicone breast prosthesis for women who had a full mastectomy. Women who had a partial mastectomy will receive $105.00
The ADP Breast Grant is mailed to the women after she has purchased the prosthesis and mailed the receipt and the Completed ADP BREAST GRant form to ADP
